San Clemente State Junior Lifeguard Registration Instructions

RETURNING JUNIOR LIFEGUARDS ONLY - Please download both pages of the enrollment application as well as the Uniform and Tuition form. Once they are completely filled out (don’t forget to sign the back) and sent in with your payment we will send your confirmation and receipt.

Please make checks out to State of California, DPR and write you license number on the check.

NEW REGISTRATION – Feel free to download both forms and fill them out but do not send them into our office. Bring them with you to the tryout to save time. You must complete the tryout prior to being registered. Rest assured that we will have a space for you.
